Level 1 services:

  • Scheduled social, recreational, & spiritual activities

Level 2 services - All Level 1 services plus:

  • Nutritional meals dealing with mechanically altered diets
  • Frequent supervision or assistance with ambulation such as walking &/or support with wheelchair mobility
  • Verbal encouragement to take a shower, get dressed, & personal grooming

Level 3 services - All level 2 services plus:

  • Minimal supervision or assistance with taking a shower, getting dressed & personal grooming
  • Minimal supervision or assistance going to the restroom such as bathing & toileting, including assistance with undergarments
  • Minimal supervision or assistance with transfers such as getting in & out of bed &/or chair or from one seat to another
  • Support with adaptive dining needs including the use of dining utensils, nutritional supervision, cutting food, use of condiments, or thickened liquids
  • Personal laundry services

Level 4 services - All level 3 services plus:

  • Guidance to maintain proper nutrition including hand-underhand assistance with dining
  • Additional care partner to assist with taking a shower, getting dressed & personal grooming
  • Additional care partner to assist with getting in & out of bed&/or chair or from one seat to another
  • Extra time to ensure the safety & security of the care partnership team or additional time for personal services.

At Prema Senior Living, we provide 7 care levels in our assisted living community and 4 care levels in our memory care community. Each resident’s care level is determined through a nursing assessment at admission, and adjusted as their need for help with Activities of Daily Living (ADLs) evolves. ADLs include everyday activities we perform to take care of ourselves, such as:

  • Bathing: Maintaining personal cleanliness.
  • Dressing: Selecting and putting on clothing.
  • Toileting: Managing bathroom needs.
  • Eating: Enjoying meals and snacks.
  • Transferring: Moving between places, such as getting in and out of bed.
  • Continence: Managing bladder and bowel functions.

Why track ADLs?
  • Recognizing needs: By tracking ADLs, we identify when a resident may be struggling with daily activities, allowing us to step in with support.
  • Evaluating progress: Monitoring these activities helps assess a resident’s well-being and adjust their care as needed.
  • Ensuring the right care: ADL tracking helps us determine the appropriate level of care, ensuring each resident receives personalized assistance, whether in assisted living or memory care.

Tracking ADLs is a crucial tool in determining the best care for our residents and ensuring they receive appropriate support through our personalized care levels.
