Levels of Care & Activities of Daily Living (ADL)

Tailored Care for Every Phase of Life

Level 1 services:

  • Scheduled social, recreational, & spiritual activities
  • Three well-balanced meals per day
  • Specialized diets & alternative menu choices
  • Weekly detailed apartment cleaning & linen service
  • Transportation for scheduled shopping, dining, & events
  • Semi-annual wellness assessments & review
  • Scheduled Transportation to medical appointments
  • Daily spot cleaning & trash removal in apartments
  • All utilities including Wi-Fi
  • On-site physical, occupational, & speech therapy
  • 24-hour access to staff with emergency call system

Level 2 services - All Level 1 services plus:

  • Nutritional meals dealing with mechanically altered diets
  • Frequent supervision or assistance with ambulation such as walking &/or support with wheelchair mobility
  • Verbal encouragement to take a shower, get dressed, & personal grooming

Level 3 services - All level 2 services plus:

  • Minimal supervision or assistance with taking a shower, getting dressed & personal grooming
  • Minimal supervision or assistance going to the restroom such as bathing & toileting, including assistance with undergarments
  • Minimal supervision or assistance with transfers such as getting in & out of bed &/or chair or from one seat to another
  • Support with adaptive dining needs including the use of dining utensils, nutritional supervision, cutting food, use of condiments, or thickened liquids
  • Personal laundry services

Level 4 services - All level 3 services plus:

  • Guidance to maintain proper nutrition including hand-underhand assistance with dining
  • Additional care partner to assist with taking a shower, getting dressed & personal grooming
  • Additional care partner to assist with getting in & out of bed&/or chair or from one seat to another
  • Extra time to ensure the safety & security of the care partnership team or additional time for personal services.

At Prema Senior Living, we provide 4 care levels within our assisted living community and 2 care levels in our memory care program. A resident's level of care is determined by a comprehensive nursing assessment upon admission and updated based on any changes in their need for assistance with Activities of Daily Living (ADLs). ADLs refer to the daily tasks we all perform to maintain personal well-being, such as:

  • Bathing: Maintaining cleanliness and hygiene.
  • Dressing: Selecting and putting on clothing.
  • Toileting: Managing personal hygiene needs.
  • Eating: Enjoying meals and snacks.
  • Transferring: Moving between places, like getting in and out of bed.
  • Continence: Managing bladder and bowel control.

Why do we track ADLs?
  • Identifying needs: Tracking ADLs helps identify when a resident may require more support with everyday tasks.
  • Monitoring progress: For current residents, it allows us to measure their progress and note any changes in their care needs.
  • Tailoring care plans: By understanding a resident’s ADL capabilities, we ensure they receive the appropriate level of care, whether they are in assisted living or memory care.

ADL monitoring plays a key role in determining the right care approach and ensuring residents receive the appropriate support through our comprehensive care levels.

Since moving our mother to the Ashton Hills memory care unit, we have been very happy with her care and the facility. It is a brand new and state of the art facility but we are treated like family. There are so many activities designed to keep her active and involved, always something fun going on. And families are invited, engaged and supported as well. The staff members are friendly, caring and do a great job of meeting the needs of the dementia patients, who are all so different with different needs. I finally feel comfortable leaving Mom because I know she is happy and being well taken care of!
